We will request your information in order to provide chiropractic treatment, in addition we will only examine or treat you with your prior explicit consent. We will from time to time need to contact you to confirm appointments by sending you text reminders and to inform you of changes to appointments, we do not need consent to do this as this constitutes legitimate interest.

Upon completion of the patient details form and consent form, all paper files and information therein will be electronically scanned and stored on computer file for as long as you remain a patient of the clinic, and thereafter for a period of 8 years. Paper records will be retained for the same period and stored in a locked filing cabinet.
As we process your personal data, you have certain rights: these are a right of access; a right of rectification; a right of erasure; and a right to restrict processing.
